Medical Documentation is the science dealing with the systematic management and dissemination of all sorts of medical information. Medical information is the lifeblood of the health care delivery system. It is often been said that an adequate medical record indicates adequate care. The information stored in the medical record will protect the legal interests of the patient, health care providers and health care facility.  As the concept of health has changed from the treatment of acute conditions to that of maintaining a state of well being, the concept of patient has also changed to ‘consumer’.  Consumers want to have a voice in selecting health care services, they want explanation of their problems and knowledge of what is going to take place in their care plans.  Health care providers are finding it necessary to reorient themselves to this more open and informed attitude.

Medical record documentation takes even greater importance in such an environment.  Grading of hospitals by national and international accrediting agencies like NABH, JCAH, ISO etc. increase the vital importance of Medical record department.  To cope with these conditions, the school of Medical Education of Mahatma Gandhi University has started Masters Degree programme in Medical Documentation. Those who completed the M.  App. Sc. Medical Documentation will be capable of dealing with total information management in hospitals and medical documentation institutions In India and Abroad.  Kerala Public Service Commission recognized this qualification for the post of Medical Record Librarian.
